- Signed Doctor's Blood Request (1 Original copy) - Secure from Hospital
- Certificate of Indigency from Barangay (1 Original Copy) - Secure from Barangay where the patient resides
- CMO Referral Slip (1 Original Copy) - Secure from City Mayor's Office - Administrative Division
